The pairing had existed in fan works for almost ten years before the two characters became the focus of story arc and began an actual romantic relationship on the show. As Cartman exhibited abusive behavior during the relationship, it was met with a mixed reaction among fans and isn't shipped by many users.
The pairing's origins can be traced back to Heidi's first speaking appearance (in which she was referred to as Marcy) in the episode "Probably", where she refused to leave Cartman's church with her parents and Cartman insists to her parents that she must stay to be "saved". In "Marjorine", Heidi acted like Cartman, with some qualities reminiscent of Cartman's original 'Whiny' personality, such as bullying Marjorine and exhibiting displeasure at the New Girl being invited to her party.
Most notably, Heidi and Cartman were paired together in the episode "Follow That Egg!" and seem to get along rather well, the only sign of displeasure being Heidi's moan of 'Awwww!' on being partnered with him. Also, Cartman pleads with Mrs. Garrison to change Heidi's grade to a well-deserved A and his to an F when he breaks his egg. Though he claims that this will cause their grade to average out to a C- for the both of them, he never voices why and he keeps his sincerity true.
They also sat together in "The Ungroundable", when Mr. Mackey was teaching computer lessons.
In the season 19 premiere "Stunning and Brave", Cartman is said to have called Heidi 'clitty litter' before being suspended by PC Principal and is forlorn and disappointed at the punishment he has received.
In Season 20, beginning in "Member Berries", Heidi is the first of the girls to accuse Cartman of being the sexually harassing troll Skankhunt42, but after continued bullying, she quits Twitter entirely. When Cartman is forcibly removed from social media and proven innocent, he and Heidi hang out together and bond over not using social media anymore. Heidi eventually apologizes to Cartman for accusing him and they become friends. When he expresses confusion about vaginas, she offers to show him one.
By the episode "Weiners Out", Heidi has shown Cartman her vagina (from six feet away, in the dark, and quickly) and they begin a romantic relationship as boyfriend and girlfriend. They use pet names like "babe" and "boo" for one another, hold hands, and both defend each other to Kyle Broflovski. Cartman frequently reminded other characters that she was both "really smart" and "really funny".
By the last few episodes of Season 20, primarily, "The End of Serialization As We Know It," Cartman experiences noticeable tension with Heidi after Kyle informed him that TrollTrace would allow Heidi to read his posts on social media, including a misogynistic post about the newer Ghostbusters film, despite neither Cartman nor Heidi still using the internet. Cartman becomes jealous when he believes Butters Stotch is trying to win her over, only for Butters' misogyny towards women after a recent breakup to spread back to Cartman, who begins to question if Heidi really is funny.
The season concludes with the tension between Heidi and Cartman remaining active as he begins to mistrust her and other women, while still re-affirming his own love for her - he even overtly wipes Heidi's kiss off of his cheek while she turns her back and walks towards her house.
They break up in the Season 21 premiere, "White People Renovating Houses", because Cartman is not interested in listening to Heidi talk about her emotions or day, or sharing with her.
In the episode "Put it Down," the two are back together, but only because Cartman threatened to commit suicide. He spins the information as if Heidi threatened to commit suicide, until she discloses a voicemail of Cartman doing so instead to Stan.
In the episode "Sons of Witches," Heidi is seen getting ready for the pumpkin patch with Cartman, despite it being already open an hour. Their date was abruptly canceled by the witch, which led Cartman to get Heidi killed off. Cartman gets Heidi to dress up as "Hanzel and Gretel," respectively, and took her unto the woods. When the witch revealed itself there, Cartman immediately ducked out, leaving Heidi to be kidnapped by the witch.
They broke up twice in "Doubling Down", since which he has been manipulating her diet and mental state, causing them to become more alike.
In the episode "Moss Piglets," Heidi is now molded into Cartman's apperance, actively expressing some of his traits to the girls at both the cafeteria and the bus stop. When Cartman wanted the old Heidi back, she simply refuses, and gets the Annual Science Fair canceled.
They were seen in a fight in "Super Hard PCness," in which Cartman wanted to see Terrance and Philip at Stan's house, while Heidi wanted a promised date night. When Kyle tried to convince everyone to "stop being so mean to each other," Heidi mocks him. Cartman immediately walked off with her, both of them being happy. They two were seen again at Stan's house, making fun of Kyle.
They broke up for the final time in the Season 21 finale, "Splatty Tomato", after Heidi realizes that she has been being manipulated and subsequently engaging in self-victimization ever since she began seeing Cartman.
In "Basic Cable" After Scott seeks relationship advice from Cartman based on his long relationship with Heidi, Cartman laments that relationships suck and that 'Girls are diabetes times 10." He also says, "But when you feel like killing yourself in two years, don't do it in front of me.", likely a reference to their relationship spanning a total of two years, and Cartman's tendancy to say he was going to kill himself in their relationship.
This pairing was never popular as a main pairing for the focus of a fanfiction, but was relatively often used in stories where Cartman's relationships are only touched upon or mentioned, generally a "throwaway pairing", and was often the second most popular heterosexual Cartman ship after Candy, though it did have some shippers. Heidi's exact characterization was somewhat variable in these fics, sometimes being treated as a similarly hateful character to Cartman, while in other instances being more comparable to the other canonical female characters, and the relationship dynamic consequently varied. Nonetheless, it declined for several years when Cartman was revealed to have attractions to men, and was becoming virtually non-existent by the time it was referenced in canon.
Since the twentieth season and it's canonization, interest in Heiman first spiked significantly, leading to a rivalry with Kyman shippers. Both people in favor of the relationship and those against it have often discussed it due its frequent usage in new episodes. However, the show's decision to depict Cartman as an abuser towards Heidi, which was re-affirmed in the final episodes of season 21, lead to a decline in interest towards the ship as many users did not want to be seen as glorifying relationship abuse and many hoped Heidi would find a safer, supportive relationship.
Since those seasons, Heidi has not spoken and fan interest has wavered, and social media users have been vocal that as it is a canonically abusive relationship it should not be included in fanfic or shipped, with many users opting to ship Heidi with other male characters.
